Medical Certificates

There are a number of different medical certificates our clinicians are able to provide. Most of these, including those for children, will incur a fee – even if an appointment is not required.

ACC Medical Certificates

ACC Medical Certificates (ACC45, ACC18 & Return to Work) require a standard appointment with your clinician and normal consult fees apply.

WINZ Medical Certificates

WINZ Medical Certificates require a standard appointment with your clinician and normal consult fees apply.

Drivers Licence Medical Certificates

Drivers Licence Medical Certificates require a standard appointment with your clinician and is charged as a normal consult fee plus an additional $20.

GP Off Work/School Certificates

GP Off Work/School Certificates require a standard appointment with your clinician and normal consult fees apply.

Nurse Off Work/School Certificates

Nurse Off Work/School Certificates can be discussed with our phone triage nurse in lieu of an appointment with a clinician. A number of workplaces, schools and early learning centres will accept these if an appointment is not necessary. These will incur fee of $30 ($20 with Community Services Card).
