Enrolments

To ensure we can provide the best care for our local community, we welcome new patients who reside within our practice boundaries, which include Rolleston, Burnham, Dunsandel, Weedons, Templeton, Springston, Lincoln and West Melton. We are unable to accept enrolments from those residing in Darfield, Kirwee, Hororata, Greendale, Leeston, Prebbleton, Rakaia or Southbridge. If you are unsure if you are eligible, please complete the form below with your address, and we will advise as soon as possible.

If you would like to enrol with our practice, please fill out the form below and we will send you the enrolment documents. You only need to complete the form once per household and can then print the number of forms required. If you are unable to print at home, please visit reception to collect an enrolment pack.

Completed paperwork must be returned to reception in person with the required identification – we cannot accept them electronically.
Once we have received and processed the enrolment paperwork (please allow ten working days for processing), we will send you a text message asking you to phone and book your Nurse Enrolment Appointment – only for adults aged 20 and over (fee of $40.00 or $20.00 with community services card).
